Mail your Prayer Remembrances to: Prayer Remembrances, Mount Carmel / Blessed Sacrament Parish, 648 Jay St., Utica, NY 13501. If you don’t have the Prayer Remembrance envelope, which are available in church and the parish office, you can download and print the form by clicking here, or mail in the following information:

- A minimum $5 donation is requested for each intention
