Due to limited help in the parish office, we ask that when submitting Prayer Remembrance requests, they are done on SEPARATE ENVELOPES for each request, and not with multiple names on one sheet of paper.
When requests are sent in with multiple names on one page, it makes for more work and the possibility of errors.
The envelopes are available in the church, or you can print a copy of them from our parish website (www.mountcarmelblessedsacrament.com) by clicking on Prayer Remembrance Forms in the right column. You also can submit your requests using one plain white envelope per each request. Just make sure you indicate whether it’s for living or deceased, the week you are requesting and from whom the intention is requested.
We kindly ask for a $5 minimum donation for each Prayer Remembrance. Please note, if you would like a loved one to be remembered each week, you need to fill out and submit a separate envelope for each of those weeks — the cost for the year is $260.
